IMPORTANT – WHAT YOU MUST DO
- Attend our appointment, and sign in person
- Bring Photo ID, which must be one of the following:
- A valid passport issued by/on behalf of a Government recognised authority, or
- National Identity Card issued by authorities of
- An EU Member State, or
- The Swiss Confederation, or
- A Contracting Party to the EEA Agreement, or
- Document equivalent to a passport
- Being issued by/on behalf of a Government recognised authority
- While establishing a person’s identity and nationality, or
- Travel document issued by the Minister for Justice (s4 of the Refugee Act 1996), or
- Travel document issued by a State
- Thus providing the holder with a document in lieu of a national passport
- Furthermore, as regards documents and forms to be signed
- Bring the entire document/form, not just the part(s) to be signed
- And complete documents/forms in advance, to the best of your ability
- Otherwise a charge may be incurred for additional time spent
- However, do not complete the section to be signed/sworn/declared
- If unsure, CONTACT ME beforehand
- To certify documents/forms as true copies, you must also bring the original
- Note – certain Affidavits can be sworn by video conference
- As per the Rules of the Superior Courts (Affidavits), 2021
- However, the Affidavit must give reasons, and follow strict procedures

EQUALLY IMPORTANT – MY ROLE AND ITS LIMITS
- Doug Lucey, Commissioner for Oaths, will not discuss or explain any of the following
- The contents, meaning or consequences of any documents/forms
- And whether or not you should sign documents/forms
- Because these are matters for discussion/decision with your legal and/or other advisers
- Furthermore, such discussion/decision must take place in advance of our appointment
- Therefore, Doug Lucey’s role, as a Commissioner for Oaths, extends to
- Verifying your identity and details
- As well as establishing that you have read and are familiar with documents/forms
- And seeking confirmation that the information you provide is correct
- While requiring that you swear/affirm/sign documents/forms in my presence
- And ensuring that correct procedures are followed
